Gout flares start at night far more often than chance. There are reasons.

The flare gets all the attention. The nights in between are where most of the difference is made.

Ask anyone who gets gout when their flares begin and you will hear the same answer. The early hours. Typically the big toe, typically severe enough to wake them, typically from nothing.

That is not folklore. Nocturnal onset in gout is well described, and there are plausible reasons for it.

What is happening

Gout is caused by urate crystals forming in a joint. Urate is a normal waste product. When blood levels stay high enough for long enough, it comes out of solution as sharp microscopic crystals, and the immune system responds to those crystals violently. That response is the flare: heat, redness, swelling and pain out of proportion to anything you did.

Key point that changes how people manage it: the flare is the visible event, but the crystals accumulate silently for a long time beforehand. What you do between flares matters more than what you do during one.

Why night

Several factors line up in the early hours.

Lower core temperature. Urate is less soluble at lower temperatures, and peripheral joints such as the big toe cool the most overnight. Less soluble means more likely to crystallise.

Overnight dehydration. Hours without fluid concentrates blood urate.

The natural cortisol trough. Cortisol is your own anti-inflammatory, and it bottoms out in the first half of the night, so inflammation faces less natural restraint.

Stillness. Reduced movement and circulation through the joint.

What actually reduces flares

Urate-lowering therapy is the main event. If your flares are recurrent, allopurinol or febuxostat, dosed to a target blood urate level and taken continuously, is the intervention with real evidence behind it. Most people who keep flaring are either not on it or not at target. If that is you, that conversation with your GP outranks everything else on this page by a wide margin.

Do not stop urate-lowering therapy during a flare. Common and counterproductive.

Hydration. Particularly before bed, given the overnight concentration effect.

Diet, in proportion. Beer and spirits, sugary drinks and high-fructose foods matter more than the traditional purine lists people are handed. Diet alone rarely gets someone to target if their urate is genuinely high, which is worth knowing so you do not blame yourself for a genetic set point.

Keep the joint warm overnight. Follows directly from the solubility point. Socks are not a treatment, but they are free.

The nights in between

Between flares most people do nothing, because nothing hurts. That is the window where the crystal load is quietly building and where sleep quality and background inflammation sit.

Published grounding research, largely from Chevalier and colleagues, has focused on inflammatory markers, sleep quality and overnight cortisol rhythm. To be clear about what that does and does not mean: none of it studied gout, none of it lowers urate, and it is not a substitute for urate-lowering therapy. Anyone telling you otherwise is overselling it.

What it is: a change to the overnight hours across the weeks between flares, requiring nothing of you once it is set up. A conductive sheet sits under your fitted sheet with a cord to the earth pin of a wall socket. No electricity, no power drawn.

If you try one

Most grounding sheets use a silver coating that tarnishes and washes out, so conductivity declines invisibly. Ours use 316L medical-grade stainless steel fibre woven through the fabric, which does not tarnish or wash out.

Judge it over weeks. The trial runs 120 nights, and if it does nothing for you, send it back for a refund.

Risk reversal

You get 120 nights to decide.

Sleep and inflammation changes are gradual. A two-week return window tells you nothing useful, which is why ours runs for 120 nights from delivery.

Use it, wash it, sleep on it every night. If it has not done anything for you, send it back and we refund it.

What it is, simply.

1
Lay it on the mattress. The sheet is woven with surgical stainless-steel thread. Goes under your fitted sheet. Takes 30 seconds. You cannot see it or feel it.
2
Plug into your wall socket's earth pin.The third pin on any wall socket runs into the actual ground beneath your house. No electricity flows through the cord. It just connects you to the Earth's charge.
3
Sleep on it.Your body reconnects to the Earth's natural electrical field for the eight hours you are in bed. It is what every body did for two hundred thousand years, until we stopped being barefoot.

Why stainless steel matters.

Most grounding sheets are made with silver thread. Silver oxidises. After 6–9 months, the conductivity of a silver-threaded sheet drops by 30–60% — and it stops grounding you properly. Premium Grounding uses surgical stainless steel, which doesn't oxidise, doesn't tarnish, and stays conductive for over a decade.

Honest questions, honestly answered.

Does it use electricity? Is it safe?

No electricity flows through the cord. The earth pin on a wall socket is connected to a copper rod buried in the ground beneath your house. The sheet uses that physical connection to bring the Earth's natural negative charge into your body — the same charge you'd touch by walking barefoot on grass. There is no current, no voltage, no battery. It's completely passive.

Will I feel anything when I plug it in?

Most people don't feel anything immediately. Your body's measurable changes (lower cortisol, higher HRV) start in the first few nights. The subjective feeling — deeper sleep, calmer mornings — tends to land in week one or two for most people.

Does it work with HRT, supplements, or other medication?

Yes. Because there's nothing chemical happening, there's nothing to interact with anything you're taking. Many of our customers are on HRT or other medications and report that the sheet works alongside whatever else they're doing.

Can I wash it?

Yes — cold machine wash, line dry. The stainless-steel thread is woven through cotton fabric and rated for 500+ wash cycles. (For comparison, silver-thread sheets typically lose meaningful conductivity after about 50 washes.)

What if my partner sleeps on the same bed?

The sheet is large enough to cover most of a queen or king mattress, and both people receive the grounding effect simply by being in skin (or thin clothing) contact with the sheet. Many couples buy one and share. Some buy two and put one on each side.

What if it doesn't help me?

Premium Grounding offers a 120-night money-back trial. Sleep on it for up to 120 nights. If it does not change anything for you, send it back and we refund it. Across 40,000+ customers, only 1.4% have sent theirs back, and the other 98.6% are still sleeping on theirs.

Why doesn't my doctor know about this?

The peer-reviewed research on grounding has only become substantial in the last 10–15 years, and most GPs aren't trained in environmental physiology. The science is real (Chevalier, Oschman, Passi have published in mainstream journals), it's just not yet part of the standard curriculum. We always recommend talking to your GP about anything new you add.
